What it is
Azelaic acid is a naturally occurring dicarboxylic acid with antimicrobial, anti-inflammatory, and antioxidant properties. It normalizes keratinization to unclog pores, calms the redness and bumps of rosacea, and inhibits the enzyme tyrosinase to reduce excess pigment. This multi-target profile makes it useful across acne, rosacea, and melasma.
Benefits
At 15-20%, azelaic acid significantly improves rosacea, acne, and melasma, with the 15% gel comparable to metronidazole for rosacea and 20% outperforming vehicle and even hydroquinone 2% for melasma in some studies. Prescription strengths are 15% and 20%, the concentrations used in the supporting trials.
When to take it
Apply a thin layer once or twice daily to clean, dry skin, building up as tolerated. It does not cause photosensitivity, but daily SPF remains advisable for any pigmentation concern.
Side effects
Common effects are transient stinging, burning, tingling, and mild dryness on application. It is generally well tolerated and considered safe in pregnancy, but irritation can occur early in use.
